Self-Assessment Tracking Tool!

I put together an Excel file to help people track their monthly progress using the assessment process that we had talked about yesterday. It is the old 97-2003 compatible format so it should be loadable in most spreadsheet applications (i.e. open office or Google spreadsheets.)
